This change removes usage of PANIC_DATA_PTR where possible. Now panic
data is accessed through functions that performs more checks and in case
of writing also moves other data when necessary.

Different versions of the linker behave differently when mixing object
built with lto enabled (desirable for code size reduction) and disabled
(assembler code), especially when they refer to each other symbols:
The file evaluation order of the linker becomes important as it
eliminates dead code at various points in time, and LTO code referring
to non-LTO code or vice versa, is not visible at early runs.
Sadly, just changing the order on the command line isn't sufficient:
What works for gcc8 breaks gcc6 (and may behave different in even more
ways on gcc4 or other versions).
Therefore, implement the vector table in C, so it's compiled in LTO
mode, just like the code it refers to.

Add CONFIG_LTO to use GCC Link-Time Optimizations to try to reduce the
flash footprint of the firmware.
Add additional protection to some functions/data to avoid removal by the
linker when their usage is not obvious.

On hard reset / hibernate, RAM will be erased and panic data will
normally be lost. When software panic data saving is enabled, try to
save this data just before hard reset and restore it when we come back
up.
BUG=chrome-os-partner:37380
TEST=Manual on Samus with WP + SW sync enabled. Boot AP, then run "crash
divzero" on console. After hard reset, verify that "panicinfo" dumps
data and shows divzero exception code.

Make non-exception "software" panics such as stack overflow and assert
failure save a panic log. Log the panic type in r4, and misc. panic data
in r5 so that panic reasons can be distinguished.
BUG=chrome-os-partner:36744
TEST=Manual on samus_pd. Run 'crash divzero' then 'panicinfo' after
reboot. Verify that panic info is printed with "r4 :dead6660". Trigger
stack overflow, verify that panic info is printed with "r4 :dead6661".

The Cortex-M0 core is based on ARMv6-M instruction set rather than
ARMv7-M as Cortex-M3 and M4.
