* einfo.ads: Clarify use of Is_Private_Primitive.
* sem_ch6.adb (Analyze_Subprogram_Declaration): An operation is a
private primitive operation only if it is declared in the scope of the
private controlling type.
* exp_ch9.adb (Build_Wrapper_Spec): Build wrappers for private
protected operations as well.
