* cppspec.c: Treat two non-option arguments as input and
output file. Three or more non-option args is an error.
Clean up.
* gcc.c (default_compilers): Pass -$ to the preprocessor.
* cp/lang-specs.h,ch/lang-specs.h,f/lang-specs.h,
objc/lang-specs.h: Likewise.
