This patch implements Jakub's idea of adding an empty struct with
a typecast operator that returns nil vectors. This is useful to
shorten all the initializers when declaring vec instances or passing
nil vectors as function arguments.

I have tested the patch pretty extensively:
- Regular bootstraps on x86_64, ppc, ia64, sparc and hppa.
- Bootstraps with --enable-checking=release
- Bootstraps with --enable-checking=gc,gcac
- Basic builds on all targets (using contrib/config-list.mk).
We no longer access the vectors via VEC_* macros. The pattern is
"VEC_operation (T, A, V, args)" becomes "V.operation (args)".
The only thing I could not do is create proper ctors and dtors for the
vec class. Since these vectors are stored in unions, we
have to keep them as PODs (C++03 does not allow non-PODs in unions).
This means that creation and destruction must be explicit. There is a
new method vec<type, allocation, layout>::create() and another vec<type,
allocation, layout>::destroy() to allocate the internal vector.
For vectors that must be pointers, there is a family of free functions
that implement the operations that need to tolerate NULL vectors.
These functions all start with the prefix 'vec_safe_'. See the wiki
page for details.
The gengtype change removes the special handling for VEC() that used
to exist in gengtype. Additionally, it allows gengtype to recognize
templates of more than one argument and introduces the concept of an
undefined type (useful for template arguments that may or may not be
types).
When a TYPE_UNDEFINED is reached, gengtype will ignore it if it
happens inside a type marked with GTY((user)). Otherwise, it will
emit an error.
Finally, gengtype rejects root types marked GTY((user)) that are not
first class pointers.

Several VEC member functions that accept an element 'T' used to have
two overloads: one taking 'T', the second taking 'T *'.
This used to be needed because of the interface dichotomy between
vectors of objects and vectors of pointers. In the past, vectors of
pointers would use pass-by-value semantics, but vectors of objects
would use pass-by-reference semantics. This is no longer necessary,
but the distinction had remained.
The main side-effect of this change is some code reduction in code
that manipulates vectors of objects. For instance,
- struct iterator_use *iuse;

- iuse = VEC_safe_push (iterator_use, heap, iterator_uses, NULL);
- iuse->iterator = iterator;
- iuse->ptr = ptr;

Compile time performance was not affected.
Tested on x86_64 and ppc64.
Also built all-gcc on all targets using VEC routines: arm, bfin, c6x,
epiphany, ia64, mips, sh, spu, and vms.

context of the macro definition. That is, the line from the first
output that is redundant with the first diagnostic line that has
line/column number:

there are cases where the first diagnostic that has a line/column
number doesn't point to a location inside the definition of the macro

Now that diagnostics for tokens coming from macro expansions point to
the spelling location of the relevant token (and then displays the
context of the expansion), some ugly (not so seldom) corner cases can
happen.
When the relevant token is a built-in token (which means the location
of that token is BUILTINS_LOCATION) the location prefix displayed to
the user in the diagnostic line is the "<built-in>:0:0" string. For
instance:
<built-in>:0:0: warning: conversion to 'float' alters 'int' constant value
For the user, I think this is surprising and useless.
A more user-friendly approach would be to refer to the first location
that (in the reported macro expansion context) is for a location in
real source code, like what is shown in the new test case
gcc/testsuite/g++.dg/warn/Wconversion-real-integer2.C accompanying
this patch.
To do this, I am making the line-map module provide a new
linemap_unwind_to_first_non_reserved_loc function that resolves a
virtual location to the first spelling location that is in real source
code.
I am then using that facility in the diagnostics printing module and
in the macro unwinder to avoid printing diagnostics lines that refer
to the locations for built-ins or more generally for reserved
locations. Note that when I start the dance of skipping a built-in
location I also skip locations that are in system headers, because it
turned out that a lot of those built-ins are actually used in system
headers (e.g, "#define INT_MAX __INT_MAX__" where __INT_MAX__ is a
built-in).
Besides the user-friendliness gain, this patch allows a number of
regression tests to PASS unchanged with and without
-ftrack-macro-expansion.
Tested and bootstrapped on x86_64-unknown-linux-gnu against trunk.
Note that the bootstrap with -ftrack-macro-expansion exhibits other
separate issues that are addressed in subsequent patches. This patch
just fixes one class of problems.
The patch does pass bootstrap with -ftrack-macro-expansion turned off,
though.

In this third instalment the diagnostic machinery -- when faced with
the virtual location of a token resulting from macro expansion -- uses
the new linemap APIs to unwind the stack of macro expansions that led
to that token and emits a [hopefully] more useful message than what we
have today.
diagnostic_report_current_module has been slightly changed to use the
location given by client code instead of the global input_location
variable. This results in more precise diagnostic locations in
general but then the patch adjusts some C++ tests which output changed
as a result of this.
Three new regression tests have been added.
The mandatory screenshot goes like this:
