Memoize Github::Shell's secret token
## What does this MR do?
`API::Helpers#secret_token` was reading the secret file on every invocation. This MR reads the file in the `gitlab_shell_secret_token.rb` initializer and saves it as a class variable at `Gitlab::Shell.secret_token`

- I'm not sure if the use of `cattr_accessor` is the best approach, or if should be moved into the `class << self` block?
- Should `API::Helpers#secret_token` be removed in favor of using `Gitlab::Shell.secret_token`?

The Sortable concern has a default scope that adds ORDER BY to all
queries. EXPLAIN ANALYZE shows that this additional ORDER BY statement
causes the SQL optimizer to use the wrong index, which leads to a load
time of 2.9 s vs 0.073 ms just for the SELECT call. The minimal
change here is to re-implement find_by using where and reorder to
remove the ORDER BY clause in IssuesController#index.

Replace bootstrap caret with fontawesome caret

1. Replaces all occurrences of `.caret` with fontawesome's caret (`fa-caret-down` for most cases)
2. Remove `.caret` from CSS
3. Refactored pipeline graph toggle to display the correct fontawesome caret (Had to change some JavaScript for this)

Only hide deploy status external link on x-small screens.
## What does this MR do?
Fixes [a regression](https://gitlab.com/gitlab-org/gitlab-ce/commit/03ea01946524a74773b24430c81804c2724b84b6#68dead621eaa630b5150a4d71b59fa3cd19030fd_57_50) where the deploy status widget hides only the text within the widget, and only for screens within bootstrap's 'small' range (≥544px - <768px).

I changed the viewport width range to hide the widget from `sm` to `xs` because when the external link is removed, it seems to fit comfortably even on xs viewports.
Given that we were already hiding the external link (along with all of the deploy text) on `sm` viewports, I assume it's okay to hide the external link still, instead of wrapping it to a new line.
We could remove the conditional hiding altogether though, and allow the external link to wrap. Definitely doesn't look as nice, but if it's important to show the external link along with the deploy status, we could make it look better when it wraps.

Looked buggy, and prevented context being given for build passed icon on smaller screens.

Prevent flash alert text from being obscured when container is fluid
## What does this MR do?
Fixes [a regression](https://gitlab.com/gitlab-org/gitlab-ce/commit/a2af7790) that causes the text within server-initiated flash *alerts* (not flash *notices*) to be obscured when `.container-fluid` is applied. It was reported in relation to a cherry-picking flash alert, but the problem occurs with all server-initiated flash alerts contained by a fluid container.

Note that as part of this fix, I combined some duplicate code and removed `.flash-text` & `.content-wrapper` selectors as they didn't seem to be necessary.
I manually tested JS-initiated Flash alerts to make sure this doesn't affect their appearance. I also grepped around for anything else that might depend on these styles. Everything checked out. But... can *you* see something this is going to break?

Affected users can't read flash alerts.

Refactor TrendingProjectsFinder to support caching
## What does this MR do?
This refactors `TrendingProjectsFinder` so it can support caching of the data. See cb7d398972d786ba7133418266fa34ae641b2497 for more details.
## Why was this MR needed?
Trending projects is quite slow, easily taking seconds to load the entire page.

== Public Projects
This finder class now _only_ returns public projects. Previously this
finder would also return private and internal projects. Including these
projects makes caching data much harder and less efficient. Meanwhile
including this data isn't very useful as very few users would be
interested in seeing projects they have access to as trending. That is,
the feature is more useful when you want to see what _other_ popular
projects there are.
== Caching
The data returned by TrendingProjectsFinder is now cached for a day
based on the number of months the data should be restricted to. The
cache is not flushed explicitly, instead it's rebuilt whenever it
expires.
== Timings
To measure the impact I changed the finder code to use the last 24
months instead of the last month. I then executed and measured 10
requests to the explore page. On the current "master" branch (commit
88fa5916ffa0aea491cd339272ed7437c3f52dc7) this would take an average of
2.43 seconds. Using the changes of this commit this was reduced to
around 1.7 seconds.

Refactor remnants of CoffeeScript destructured opts and super into ES6
## What does this MR do?
Gets rid of some CoffeeScript remnants that are difficult to work with, and updates them to use ES6 syntax/constructs. I updated nearby code to ES6, but these updates usually just included using the es6 class syntax and scoped variable keywords.
**Important**: For the most part, I tried to avoid refactoring the design of the existing code. If I attempted that, this would've taken much much longer and it would've been out of this issue's scope.
## Why was this MR needed?
The compiled coffeescript for destructured default params is very difficult to work in.
These changes makes some of these rough patches more maintainable:
