(__GMP_DEFINE_TERNARY_EXPR2): Remove for now.
(struct __gmp_ternary_addmul2): Likewise. (struct __gmp_ternary_submul2): Likewise. #include <cstring>. (struct __gmp_alloc_cstring): Prefix strlen with std::. Remove mpfr code.
