Fix R6RS 'fixnum-width'.

* module/rnrs/arithmetic/fixnums.scm (fixnum-width): Rewrite to avoid
inexact arithmetic, and correct the off-by-one error.
* test-suite/tests/r6rs-arithmetic-fixnums.test (fixnum-width): Add
tests.
