cmm: Eliminate orphan Outputable instances
Here we reorganize `GHC.Cmm` to eliminate the orphan `Outputable` and
`OutputableP` instances for the Cmm AST. This makes it significantly
easier to use the Cmm pretty-printers in tracing output without
incurring module import cycles.
