cipher: setup bulk functions at each algorithms key setup
* cipher/cipher-internal.h (cipher_mode_ops_t, cipher_bulk_ops_t): New. (gcry_cipher_handle): Define members 'mode_ops' and 'bulk' using new types. * cipher/cipher.c (_gcry_cipher_open_internal): Remove bulk function setup. (cipher_setkey): Pass context bulk function pointer to algorithm setkey function. * cipher/cipher-selftest.c (_gcry_selftest_helper_cbc) (_gcry_selftest_helper_cfb, _gcry_selftest_helper_ctr): Remove bulk function parameter; Use bulk function returned by setkey function. * cipher/cipher-selftest.h (_gcry_selftest_helper_cbc) (_gcry_selftest_helper_cfb, _gcry_selftest_helper_ctr): Remove bulk function parameter. * cipher/arcfour.c (arcfour_setkey): Change 'hd' parameter to 'bulk_ops'. * cipher/blowfish.c (bf_setkey): Change 'hd' parameter to 'bulk_ops'; Setup 'bulk_ops' with bulk acceleration functions.
