This adds a `-Wunused-result`-proof `GIT_UNUSED()`, just to demonstrate
that it works. With this, sortedcache.h is now completely
`GIT_WARN_UNUSED_RESULT`-annotated!

Our file utils functions all have a "futils" prefix, e.g.
`git_futils_touch`. One would thus naturally guess that their
definitions and implementation would live in files "futils.h" and
"futils.c", respectively, but in fact they live in "fileops.h".

Rename the files to match expectations.

When confronted with a conflict in the index, `git_index_add_all`
should stage the working directory copy.  If there is no file in the
working directory, the conflict should simply be removed.
