PCI: Convert Mellanox broken INTx quirks to be for listed devices only
Change Mellanox's broken_intx_masking() quirk from an "all Mellanox
devices" to a quirk for listed devices only.
[bhelgaas: remove #defines, reorder to keep other quirks together]
