MDEV-297: SHOW EXPLAIN: Server gets stuck until timeout occurs while executing SHOW
INDEX and SHOW EXPLAIN in parallel
- Rework locking code to use the LOCK_thd_data mutex for all synchronization. This also
fixed MDEV-301.
