docs: improve discoverability of Code of Conduct
- move CoC from CONTRIBUTING to top-level, separate COC file
- add note/link in CONTRIBUTING
- add note/link in README (both at top, and newcomer resources)
- move CoC section in CONTRIBUTING to top of file
