Add support for Network QoS rule commands

  - network qos rule create --type minimum-bandwidth
                                   dscp-marking
                                   limit-bandwidth
  - network qos rule delete
  - network qos rule list
  - network qos rule set
  - network qos rule show

Fix Octavia gate breakage caused by _get_columns()

The above mentioned function tries to extract keys() from an item which
is type class.

This patch fixes the issue by converting item to dict by using
to_dict().

Update default nova api version to 2.1

We should use 2.1 as the default nova version now

The API_MIN_VERSION of novaclient has already changed to 2.1

move unit tests to new "unit" test module

this will better isolate the unit tests from the functional tests.
unfortunately, the "integration" tests had to be lumped into the
"unit" tests since we need the separation in testr.conf
