Podify the remaining README.platform files;
merge README.plan9 and plan9/perlplan9.pod;
delete plan9/perlplan9.* (the perlplan.doc needs to
be regenerated in Plan 9); make the =head1 and =head2
in the README.platform to be a little more verbose
(skipped README.os2 not to anger Ilya) so that they
look better in the toc; regen toc.
