Now completely unexecuted source files can be included in reporting.  Specifying --source tells coverage.py where to search for files that haven't been executed.

Calculate the pid suffix for data files at the end of the process so that programs calling os.fork will collect data from both child and parent. Fixes issue #56.

Reduce the amount of data translation by having the tracers record data in a form more like it will be consumed.  Also should reduce the amount of work the tracers have to do.
