Transform DocBook to HTML.

This top-level stylesheet includes all the necessary stylesheets to transform DocBook documents into HTML. It imports db2xhtml, overrides parameters, and sets a namespace alias to output non-XML HTML. This stylesheet sets html.xhtml to false.
