dfa: prefer idx_t for indexes
* lib/dfa.c (mbs_to_wchar, state_index, dfaoptimize, dfaanalyze)
(icatalloc, enlist, allocmust, dfamust):
Prefer idx_t to size_t for indexes, and use idx_t-related allocators.
