[man pages]: Improve ellipsis usage in synopses.
* Use non-breaking adjustable space escape sequence \~. This groff
extension is widely supported[1].
* "Bind" this escape sequence to adjacent roman, not italic, text. It
doesn't make a difference as far as I know; this is purely for
consistency.
* Use non-printing input break escape sequence \& only where necessary.
* Rename metasyntactic operand from "input-file" to "file".
* Mark optional file operands with brackets where they were missing; if
given none, these commands will happily read from the standard input.
