* font/devhtml/R.proto, font/devutf8/R.proto: Add `18', `38', `58',
`78', `<<', `>>'.
* font/devutf8/NOTES: Updated.

* src/roff/troff/charinfo.h (charinfo): Add `setx_macro' function.
Don't give default parameter to `set_macro'.
* src/roff/troff/input.cc (do_define_character): Use `setx_macro'
instead of `set_macro'.
(charinfo::setx_macro): Implement it.
(charinfo::set_macro): Don't change `mode'.

* tmac/tty.tmac: Add `18', `38', `58', `78', `<<', `>>'.
* tmac/ps.tmac, tmac/X.tmac: Add `<<', `>>'.
* tmac/dvi.tmac: Define `!=' with `.schar'.
