Add a 'distclean' Makefile top-level target
This cleans up a little more than "make clean",
in particular removing files generated from templates,
and automake-generated files in iscsiuio.
Also, a ".PHONY:" target was added where needed
so that each Makefile has a complete list of
phony targets.
