hv_func.h - change _PERL style macros to PVT_ style macros
C reserves identifiers starting with underbar, but I didnt realize that this applies to defines as well when I set up this file. This renames the private macros to start with PVT. It maintains the distinction between _PERL and __PERL by converting them to PVT_PERL and PVT__PERL.
