document using fetch() with Oracle
We implemented working FETCH support, but it's not
yet implied by limit/offset. The docs make no mention
that this is available which is very misleading including
to maintainers. Make it clear that fetch() support is
there right now, it's just not yet implicit with
limit/offset.
