- sqlalchemy.test and nose plugin moves back to being entirely
outside of "sqlalchemy" and under "test/".
Rationale:
- coverage plugin works without issue, without need for an awkward
additional package install
- command line for "nosetests" isn't polluted with SQLAlchemy options
