CppTools: Rename files to comply with coding standard

Rename ModelManagerInterface.(h|cpp) to cppmodelmanagerinterface.(h|cpp).
Rename TypeHierarchyBuilder.(h|cpp) to typehierarchybuilder.(h|cpp).
