Fix documentation configurations
- Fixed online documentation builds to use a dedicated .qdocconf file,
which exists but was not selected for use.
- Removed unused doc/images directory, and fixed image paths and
example image extensions in .qdocconf
- Used the configurations for Qt module docs, with some modifications.
- Renamed index.html to avoid filename conflict
